Intake/Discharge Specialist

NHS Human Services are looking for an Intake/Discharge Coordinator to join our Nazareth, Pa Team.

+ The Intake/Discharge Coordinator functions as the primary liaison to all community hospitals and other social services, mental health centers, intensive case-management units, with regard to, any referral for admission and/or discharge planning and follow-up contacts.
+ The Intake/Discharge Coordinator conducts pre-admission screenings, coordinates the referral process with county agencies, conducts continued stay reviews with managed care organizations, initiates the process for accessing financial resources such as SSI and MA, functions as an integral member of the interdisciplinary treatment team, assists in ensuring program is meeting performance quality standards, and other related duties.

+ Bachelors Degree in Social Work preferred.
+ Minimum of 2 years experience as case manager
+ Or Bachelors Degree with a minimum of 4 years clinical experience working with individuals with mental illness.
+ Valid PA Driver's License and access to reliable transportation.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
